Your name, email address, telephone number if you give one, where you are staying, your dates, the equipment you want and anything you write in the message box. We also record that you ticked the two confirmation boxes.
The eligibility box is a yes or no confirmation. We do not ask you to describe a medical condition through this website, and you should not send us one. Where the ordinance requires documentation to be checked, that happens in person when the equipment is handed over.
Why we hold it
To answer your request, to arrange the hire, and to meet the record keeping the local ordinance requires of a hire company. We do not sell it and we do not use it for advertising.
Who processes it
